Fair Entry Forms
The following Entry Forms may be filled out online and submitted to the Fair Office via e-mail. Use the tab key to move from field to field and remember to complete all necessary fields. By submitting electronically, the Fair accepts a parent’s/guardian’s typed name as their signature.
NOTE: Horse participants must identify all classes they wish to enter. If you submit a hard copy, please circle two (2) classes in which you will be receiving premiums. If you are submitting electronically, please identify the two (2) classes in the body of the e-mail. If two (2) classes are not specified, NO PREMIUMS WILL BE PAID. There are no exceptions to this policy.
When Entry Form is complete, save to a file on your computer. Attach file to an e-mail and send to kim.zills@ces.uwex.edu. Once the e-mail has been sent, you will receive a confirmation e-mail that the Entry Form has been received. It will be the responsibility of the exhibitor to make sure this confirmation has been received. Please contact the Fair office if an e-mail confirmation has not been received 1-2 business days after Entry Form submission.
